If you are 15 years of age or older, there are three ways in Oregon to sign up on the organ, eye, and tissue donor registry:

Ask the DMV to put a “D” restriction code on your license, permit or identification card.
- Go online at donatelifenw.org.
- Request a paper form by calling 1-800-452-1369.
It is extremely important that you inform your family of your wishes to be a donor.
By signing up on the Donor Registry, you are confirming your commitment to be a donor and ensuring your decision is known and honored by your family and the organ, eye, and tissue recovery agencies.
