2008 Special Tdap Project



The Oregon Immunization Program has started a project promoting Tdap vaccinations statewide. This project will continue into 2009. Tdap will be made available to interested providers at no cost. This vaccine can be used to expand no-cost Tdap to persons not eligible for VFC vaccine.

  • Tdap Vaccine Ordering Form - Print this file and fax or mail to the Immunization Program to make your order for Tdap Special Project Vaccine. Remember, this project is to expand our reach, so the Tdap ordered on this form should reflect the number of doses you hope to give above your usual patient load.
  • Tdap Special Doses Reporting - This document will outline the various methods available for providers to enter Tdap Special Project into the ALERT Registry. Reporting Special Project doses is very important and is a requirement of participation in the project.
  • Tdap Special Project VAR - This is a customizable Vaccine Administration Record (VAR) tailor-made for Tdap vaccination. Add your clinic information and it's ready for print. The front contains the minimum data needed to sumit doses to ALERT Registry and acts as a consent form. The back page is made to be used as a screening form for contraindications/precautions.
  • Adacel labels - This is a pre-filled label sheet (Avery 5160) you can use to reduce the burden of filling out VAR's. Use the 'find & replace' function to change the Lot # from 0000 to the Lot you are using. Place the labels to the far left of the 'clinic info' section so all that needs to be hand-entered is the vaccination site and the administrator's name.
