Description
This web site presents health care professionals who encounter actual or potential medication errors with several options by which to report these incidents confidentially to U.S. Pharmacopeia (USP). Potential medication errors include misinterpretations, miscalculations, misadministrations, misunderstanding verbal orders and difficulty in interpreting handwritten orders.
