Blank Window
If a Portable Document Format (PDF) form or other publication does not download correctly, leaving a blank window within your Web browser, follow these steps to resolve the issue:
  • Select a form version not marked "fillable" and click on the link with your right mouse button;
  • In the menu that pops up, select "Save Target As";
  • You should then be prompted to choose a location to save the file;
  • After choosing the location, click on the "Save" button;
  • Navigate to the file on your computer and open it; and
  • You should now be able to print the form and fill it in manually.

Bounced To Another Page When Accessing A Form Link
Some users have reported that they are transferred to another page when attempting to access a link to a form. The page they are redirected to most often is our Web Site Feedback/Suggestion Form page.
 
This is an intermittent problem that our internal technicians and our external support vendors have not been able to isolate to a single cause, or even a few circumstances. It seems that the issue is primarily with network and security setting combinations but they vary greatly depending upon the user's connection type (dial up, DSL, cable modem, etc.), operating system type and version (Windows XP, Vista, MAC OS X, etc.), and browser type and version (Safari, Internet Explorer, Mozilla Foxfire, etc.).
 
We regret this inconvenience but until a solution can be found, you may contact us via our Web Site Feedback/Suggestion Form and tell us that you are having difficulty accessing a form link on our web site and request that we send you the form via email or the U.S. mail. Be sure to tell us which form you need, how you would like us to send it and your contact information.

Fillable Forms
General Instructions
DMV offers selected forms in a "fillable" Portable Document Format (PDF) format. You may fill out these forms on your computer, print them and mail or take them to DMV.
 
Please note that it is not possible to submit these forms electronically to DMV.  You must print them out after entering in the information. If the form requires a signature, DMV must receive the form with an original signature.
 
Please print and mail or bring to a DMV Office, ALL pages of the form.
 
Some printers may require use of the "Shrink to Fit" printer dialog box to be checked in order to print on a single page.
 
When submitting a form to DMV, please print it on plain white paper only. Do not use colored, textured or patterned paper.
 
Note: If you go to an office, this will become your receipt or temporary registration.

Entering Data Into A Fillable Form
When entering data into a fillable form:
  • Mouse-click in a text box and a blinking cursor will appear.
  • Press Tab to go to the next field.
  • Press Shift+Tab to return to previous field.
  • To insert a checkmark into a checkbox, use the spacebar or mouse-click in the box.
  • Before printing, tab out of the field in which data was last entered.
  • To print, click on File and select Print. Make sure the checkbox "Print as Image" is checked.
  • All DMV fillable forms have a CLEAR FORM button that removes all of the data you have entered and resets the form.

Trouble With A Fillable Form
If you are unable to open a fillable form, an available work around is to select the version not marked "fillable" and do the following:
  • Mouse click on the link with your right mouse button;
  • In the menu that pops up, select "Save Target As";
  • You should then be prompted to choose a location to save the file;
  • After choosing the location, click on the "Save" button;
  • Navigate to the file on your computer and open it; and
  • You may now print the form and fill it in manually.
