| LICENSE VERIFICATION |
|
Do you need a License Verification?
There is NO charge in Oregon for license verifications.
The online license verifications does constitute a "primary issuing source" for purposes of license verification. You can click on the Last name, First name, License Number, or City, or scroll down to the second box for specialty areas.

You can also request a verification of licensure, by telephone at 971-673-0198, by fax at 971-673-0226 or by e-mail: Felicia.M.Holgate@state.or.us or by writing to the Board office.
We sent verifications out within a few days. The Board verifies the license number, whether it is current, the date of initial licensure, and whether the board has disciplined the licensee. We will notify you by e-mail when the verification is sent if you have your e-mail.
Note: Oregon law requires all information obtained during a complaint be kept confidential, including the fact that there is an on-going investigation (except in rare instance where there is a showing by clear and convincing evidence that disclosure is in the public’s interest). However, once the investigation is complete and the Board, by a majority vote, has taken disciplinary action, a copy of the action is available on request. These requests generally come from other state licensing boards, insurance carriers, and potential employers. See more information on the Complaints/Investigation tab.

| NUMBER OF LICENSEES |

As of September, 2008 there are just over 1500 licensed Occupational Therapists in Oregon. Included in that total are 241 OT Assistants.

| LICENSEE LIST REQUEST |
|
Licensee lists are public information and available upon request.
Licensee lists are public information and are available upon request for a fee of $25. Persons may order a list of licensees by sending in a written request, stating whether list is to be in zip code order or alpha order, along with the $25 payment.

Printed public information (minutes, rules, statutes, policies, etc.) is available upon request. The first ten pages are free. Additional pages are $ .25 a page, with additional charges if the request requires significant staff time and research.

| SUPERVISION STATEMENT |
|
OT Assistants working in Oregon must have a signed Statement of supervision
Look under Supervision or for a Statement of Supervision form click here.
A new rule deletes the requirement that the Statement of Supervision be notarized.

If it is a name change be sure to include a copy of the legal document that changed your name and exactly how you wish it to appear (do you want both names, a middle initial ?). The document can be a copy of the divorce paper, marriage certificate or name change document.
