FHIAP Direct Deposit Information for Group Members
 
To sign up for direct deposit for your subsidy check, download and complete these forms:
Both forms MUST be filled out for direct deposit to work!
Fill out Page 1, read Page 1-4 instructions and fill out the Direct Deposit Authorization form.


 
Please DO NOT fax these forms; original signatures must be on file with the Department of Administrative Services. Do not send forms to FHIAP. This is handled by a different state agency.

Be sure to have your financial institution complete and sign Section C of the Authorization form before mailing it to DAS.
 
How long will this take?
Once both forms are received by the Department of Administrative Services, setting up the connection between your financial institution and DAS takes approximately six to nine banking days. Meanwhile, you will continue to receive your subsidy payments by warrant/check, as usual. DAS will notify you of any problems.
 
Direct deposit will not necessarily speed up the process of receiving your subsidy reimbursement. It takes approximately 2-3 banking days from the date your payment is processed for the money to reach your account.

Change of Address

If you change your address, you must tell the Department of Administrative Services (DAS). Even though FHIAP knows your new address, we can't tell DAS for you. DAS handles your subsidy checks and only you can contact DAS to make changes.
 
To change a mailing address, send this information to DAS at the above address:
 
  • Your Social Security Number
  • Your name (print)
  • Your new mailing address, and
  • Your signature. The signature must be the person who signed the original authorization form. For security reasons, this request has to be an original; copies or faxes are not accepted.
