For all LCSWs and CSWAs who were born in July, please note that your renewal forms were mailed from the Board office on Monday, June 2, 2008. You should have received your renewal information a long time ago. Hopefully, you not only received your renewal form but have already renewed your License or Certificate. If so, disregard the rest of this notice.

If you have your renewal form and simply have not filled it out and sent it in with the appropriate renewal fee, you now owe a $50 late fee in addition to the normal renewel fee. Also please note that if your renewal is not “filed with and accepted by the board” by July 31, 2008, your License or Certificate will Lapse. [OAR 977-020-003(4)]
The board suggests you immediately renew your license of certificate. Please fill out and send in the form and the required fees TODAY.
