You can either fill this form out online or print it out to fill out using dark ink.
 
We highly recommend you read the instructions completely before filling out your application. If you intend to print the form out to fill it out in ink, you should also print out the instructions so you can refer to them as you complete your applications.
 

Filling out the forms
How to fill out the application electronically
Use your Tab key to go from one line or box to the next. For example, after entering your Social Security number, hit Tab, and the cursor will automatically go to the line for your name.
 
Depending on the choices you make, you may not need to fill out every section. (Read the instructions to make sure you are filling out the correct sections.)
 
When you reach a section that you do not need to fill out based on your choices, hit the Tab key until you reach the next section you need to complete.
 
You will see that the name of each section appears in blue. If you click on the blue text, the instructions associated with that section will appear on your screen.
 
You cannot save information entered in this form unless you have Adobe Approval or the full version of Adobe Acrobat software. All information will be deleted when you close this form, so make sure you print out a copy of your application for your records.
