AFIX Assessment: Preparation

Preparing for an Assessment

  1. Know the parameters of your assessment
    • Assessment Method
    • Age range and Size of Sample
    • Demographic variables and antigens to be collected
    • Active Patient definition
  2. Contact the office to schedule the assessment. If you do not have a contact name ask for the office manager.
  3. Provide the contact/office manager with background information about AFIX and the process of the Assessment and Feedback session.
  4. Have your master schedule in front of you. Offer 1-2 possible dates for assessment.
  5. Outline your workspace requirements and duration of assessment and feedback activities if conducted on the same day.
  6. Once the assessment is scheduled obtain the following information:
    • Contact Name and direct number (may also want to obtain a "back-up" or secondary contact name)
    • Confirm street address and driving directions
  7. Send to the office contact a confirmation fax or letter containing the following information:
    • Office contact
    • Date and time of assessment
    • Office responsibilities (charts pulled, workspace requirements)
    • Your contact information
    • Call office 1-2 days before assessment to reconfirm
    • Conduct Assessment
