Date: Enter the current date.

Location: The community and state where the gage is located.

Stream: Enter the precise name of the river, creek, or fork of a river/creek, that the gage is located on. The precise name can be clarified by the cooperator if there are any uncertainties.

Basin: This is simply the basin that the gage is located in. In most instances, enter the name of the major basin subdivision (i.e. Snake, Sacramento, etc.) and the major system (i.e. Malheur River, Feather River, etc.).

HSA: Enter the name of the WFO with HSA responsibility for this gage station.

References: List the sources that were used to provide information for the contents of the E-19. The list could include local, state, and federal agencies, private entities, utility companies, etc.

Location Identification: This is the National Weather Service Location Identifier (NWSLI) for the site.

NWS Index Number: This is the number that has been assigned by the National Data Climate Center (NCDC). Cooperative stations used by the NWS, that collect river or river/rainfall data, will have designated index numbers. Not all river forecast points will have these numbers (only those that are official cooperative stations).

USGS Number: Any river gage operated by the USGS, either presently or previously, has an assigned eight digit identification number.
