How to Apply
NOTE: For the September 1-15, 2008 application cycle, the IHS Integrated Diabetes Education Recognition Program is only accepting applications for Level 2 accreditation.

NOTE: The final Level 2 Application Form and Instructions are now available in Word format (8/28/2008). Due to techincal problems, a PDF fill-able form will not be available for the September 1-15, 2008 application cycle. You must submit your applcation using the August 2008 version of the Word form.


If you are interested in applying for IHS Diabetes Education Program Recognition (Level 2) accreditation for your DSME program, please read the information and instructions on pages 1-5 of the “Level 2 Application Form and Instructions” below very carefully. Per the instructions, you should read or use the other documents listed below as you prepare your application.
Deadlines
As outlined in the “Level 2 Application Form and Instructions”, the steps in the application process and deadlines for each are listed in the table below. Note that applications are accepted twice at year, on the dates outlined below.

Submit a Letter of Intent and “Level 1 Developmental Checklist”
Must be received by:
February 15th (5pm MST) – for the March 1-15 application cycle
August 15th (5pm MDT) – for the September 1-15 application cycle

Prepare and submit Application and supporting documentation
Must be received by:
March 15th (5pm MST) – for the March 1-15 application cycle
September 15th (5pm MDT) – for the September 1-15 application cycle

Notification of review outcome
Within 12 weeks of application deadline

Review Process
Your application will be reviewed by a panel of reviewers appointed by the IHS Division of Diabetes. They will use the Integrated Diabetes Education Recognition Program checklists to evaluate your program.
