Background: The goals of the Utah WISEWOMAN program for the five-year project period are to maximize the reach of the program by providing services to as many Utah women as possible; work to eliminate health disparities by serving those most in need, decrease heart disease and stroke risk factors of Utah’s WISEWOMAN population; maximize the number and variety and settings that deliver WISEWOMAN screening, risk reduction counseling, and lifestyle interventions; ensure that the program is delivered as intended; sustain the benefits of WISEWOMAN over time at both the individual and organizational level, and create a system change that will ensure that women screened in Utah’s Breast and Cervical Cancer Early Detection Program (BCCEDP) will be simultaneously screened in the Utah WISEWOMAN program.

Target Population:  Uninsured and underinsured women between the ages of 40 and 64 who live at or below 250% of the Federal Poverty Level.

Lifestyle Intervention:  A New Leaf: Choices for Healthy Living.
