The data were downloaded from http://www.medicare.gov/NHCompare/Include/DataSection/Questions/SearchCriteria.asp in March 2005.

Excludes admission assessments, residents with comatose status, or residents with missing values. Excludes facilities with fewer than 30 residents in the denominator.
