Government Vehicle Request Form


Reservations will be made in accordance with S.P.I. 5-05 guidelines. Please note: For periods longer than three days, the request must be followed by a memorandum from the Department Chair or Division Manager explaining purpose of trip, length of time out of pool, and project/activity to be charged.

Reservations may be made by completing this form and submitting it, or you may contact the BNL Transportation Office at transportation@bnl.gov , X2535 or FAX X6167.
