Remember: THERE WILL BE NO GRACE PERIOD. There will be no extensions of licensure. If you do not have your Certified Oregon Pharmacy Technician license in hand, YOU CANNOT WORK AS A PHARMACY TECHNICIAN AFTER SEPTEMBER 30, 2008.
 
Act now to avoid a lapse in your license! You may not have your license by October 1st if we don’t receive your completed application and attachments before August 8, 2008.

To expedite processing of your application, be sure to include the following:
 
  • A copy of your driver’s license or state ID,
  • A copy of your national certification or the letter stating you have passed the exam,
  • A new, original passport regulation photograph taken within the last 6 months,
  • A check or money order for $35.00.
 
 

Read the instructions on the form carefully.
 
Truthfully answer the moral turpitude questions on pages 2 and 3. If you have already provided documentation for any “yes” answers to the Board, you may write in that we have the records on file, and you do not need to provide it again. You do have to disclose (answer “yes” for) any and all old incidents again.
 
Please allow 6 to 8 weeks’ processing time for your license. We must be able to verify your national certification on PTCB’s or ICPT’s websites. Be aware that it can take 8 weeks or more before your certification may be posted on PTCB’s or ICPT’s websites. Plan ahead, and test early! Your application will be held until we are able to verify your certification.
 
 
EXAM DEADLINES, PTCB:
  • The application deadline for this April - June testing window was April 4, 2008.
  • If you test with PTCB after this window, it is unlikely that you will receive your certification before your license expires.
 
EXAM DEADLINES, ICPT:
  • It is recommended that you test by the end of June, 2008 to allow time for the posting of your certification on ICPT’s website, and the processing of your new license.

CE INFORMATION FOR CERTIFIED PHARMACY TECHNICIANS: 
The Board will accept one hour of CE law from either the Accreditation Council for Pharmacy Education (ACPE), Continuing Medical Education (CME)  or Board approved programs.  The law CE programs may contain information on State or Federal regulations such as DEA, FDA, HIPPA or Medicare part D.  Some other suggested websites to visit are www.powerpak.com, www.cecity.com and www.rxschool.com.  The Board of Pharmacy also has the one hour CE law exam available on our website.

Renewal information for the 2008-2009 CPht renewal cycle was mailed June 19, 2008.   If you currently hold a Certified Oregon Pharmacy Technician License please do not complete a new application.  

Please note, if you do not renew your license prior to the August 31st postmark deadline, you will need to include the $20.00 delinquent fee.  The delinquent fee applies to all applications postmarked on September 3, 2008 or later. Please contact our office if you need a duplicate renewal form.

Once you submit your payment and renewal form, your cancelled check is your receipt.  You, or your employer, can check the status of your license on our real-time, online verification system.
 
If you have had an address or employment change since the last renewal cycle, and you haven´t updated your information with the Board, please submit an address change form, or submit your information online. Be advised, any change of address, or change of employement must be submitted to the Board within 15 days of the change.

Please note, if you have not yet renewed your license, you will need to include the $50.00 delinquent fee.  The delinquent fee applies to all applications postmarked on June 1, or later. Please contact our office if you need a duplicate renewal form.

PLEASE NOTE:   There are two new questions on your pharmacist renewal application.  If you are currently licensed as an Oregon preceptor, please mark “yes” or “no” if you wish to renew.  If you are not licensed as an Oregon preceptor and are employed outside the State of Oregon, leave this section blank.  It is the Oregon Board of Pharmacy’s policy not to issue preceptor licenses to pharmacists who are not currently working in the State of Oregon as this license is only valid within the State.
 
Preceptor licenses will convert to a 1 year license with an expiration date of June 30th.  If you mark “yes” to have your Oregon preceptor license renewed, you will be issued a new license showing the new expiration date of June 30, 2009 even if you have previously been granted a license with an expiration date of 2009, 2010 or 2011.

Renewal forms for 08-09 were mailed the week of  January 14, 2008.
 
If your renewal was postmarked after February 28, 2008 you  are now subject to the delinquent fee as noted on your renewal form.   
 
If you have not received your renewal notice,  contact our office at 971-673-0001 or email us at pharmacy.board@state.or.us.  We will not be mailing duplicate forms unless requested.

Renewal forms for 08-09 were mailed the week of  November 5, 2007. Your location may not sell any nonprescription medications after January 31 until your registration has been renewed.
 
If your renewal is postmarked after December 31, 2007 you will be subject to the delinquent fee as noted on your renewal form.   
 
If you have not received your renewal notice,  contact our office at 971-673-0001 or email us at pharmacy.board@state.or.us.  We will not be mailing duplicate forms unless requested.
