NOTE:  Reciprocity requirements will change once the new law takes effect January 1, 2008:  Proof satisfactory to the Board that the applicant is licensed in good standing in another state and has practised as a funeral service practitioner or an embalmer in another state for three of the five years immediately preceeding the application date.  per SB 545 Enrolled 2007 Session (Old law stated two years immediately preceeding the application date.)

26. Cemetery, Change of Ownership Application   (A Background Information Questionnaire needs to be completed and attached to the cemetery application for each identified principal on the application.  If any of the proposed principals currently holds a personal license issued by the Board, the proposed principal does not need to submit the Background Information Questionnaire.)
 
27. Cemetery, Initial Application   (Please attach a completed Cemetery Licensing / Registration Questionnaire  form.  Note that a  Background Information Questionnaire will also need to be completed and attached for each identified principal.  If any of the proposed principals currently holds a personal license issued by the Board, the proposed principal does not need to submit the Background Information Questionnaire. )
