Out-of-State-Applicants
If you are planning to move to Oregon and are currently licensed or have a certain amount of professional experience in another state, chances are you will be able to gain licensure in Oregon quickly and easily if you meet Oregon qualification requirements.
 
Just be sure to review the specific requirements for out-of-state applicants described below.  We want to assist you in your successful move to our state!
 
Affidavit of Licensure
Applicants holding an active license in another state or country in which the Oregon Health Licensing Agency (OHLA) recognizes the education, examination and licensing requirements must provide an original Affidavit of Licensure mailed directly to OHLA from the licensing state with an official seal in a sealed envelope. If the seal is broken the document is considered invalid.
 
Or, applicants may have the Affidavit of Licensure electronically transmitted to OHLA from the licensing state. Preauthorization for electronic transmission is required.
 
OR
 
Experience Equivalency
Applicants working in another state or country where the practice is unregulated in which OHLA determines the work experience is equivalent to minimum requirements must provide documentation in the form of tax returns, authenticated by the Internal Revenue Service, verifying two years full time or four years part time work experience in the field of permanent color or tattooing. Additional documentation may be requested to substantiate qualification.
