Applying For Licensure in Oregon
WHEN TO APPLY:  An application for licensure may be submitted any time after the degree award date is posted on an official transcript. 
 
APPLICATION FEE:   $300 (non-refundable)
 
WORKING IN OREGON BEFORE LICENSED:
To provide psychological services in Oregon prior to being licensed, a person must be:  1) an approved applicant in a Board-approved contract for supervision; or, 2) in an exempt setting (see ORS 675.090); or, 3) if licensed in another state, must be granted a limited permit (see OAR 858-010-0055).

Your completed application and all supporting documents must be received prior to a formal review of your application.
 
A COMPLETE APPLICATION PACKET INCLUDES:

A) OBPE Application Form
The application form will become a permanent part of your licensure file.  Please respond to each question.   If a question is not applicable please indicate “n/a.”  Resumes or vitaes will not replace the Board’s application form.

B) University Director of Training - Reference Form  
Reference forms must be signed by the reference across the envelope seal and mailed directly to the OBPE office.

C)  Professional Reference Form
You must submit one form for each individual who served as a supervisor or Internship Site Director during your pre-doctoral and/or post-doctoral supervised work experience hours.  If applicable, documentation of supervised work experience can be obtained from your licensure file in other states.  You must also submit three (3) additional Reference forms from colleagues (other than supervisors) who can attest to your professionalism and fitness to practice psychology.  Reference forms must be signed by the reference across the envelope seal and mailed directly to the OBPE office.

D)  Non APA School or Universities ONLY:  Educational Record in Psychology Form  AND
University Accreditation Form:
If your school was APA approved at the time you received your degree, you DO NOT need these two forms.  These forms are only for applicants that attended a regionally accredited institution.
 
E) Resident Contract  Please submit a contract proposal if you plan to provide psychological services in Oregon prior to becoming licensed.  The contract becomes effective once your application for licensure has been approved, and the Residency contract has been approved.  Both you and your supervisor will receive a signed copy of the contract.  At the conclusion of the contract, the supervised hours are documented on the Record of Hours of Supervision Received form.

G) Transcripts (graduate work only)
All transcripts must be sent directly from the educational institution to the Board’s office.

H) Fee ($300)
A check or money order must be mailed to our Portland banking address.
 
IF YOU ARE LICENSED IN ANOTHER STATE, also include:
 
I)  License Verification
Please request verification “good standing” from any and all state(s) in which you are or have been licensed.  Verification must be sent directly from the other licensing agency to the Board’s office.

J) Supervised Work Experience documentation
Documentation of supervised work experience can be obtained from your licensure file in other states.  Documentation must be sent directly from the other licensing agency to the Board’s office.

K) EPPP (Examination of Professional Practice in Psychology) score
Must be sent directly from either the testing service or another jurisdiction.
 
IF YOU POSSESS A VALID ASPPB CPQ CERTIFICATE:
Request ASPPB send a copy of your entire file and verification of your certification directly to the Board’s office.  CPQ candidates only need to complete the Application form through item #5 (Character and Fitness).  You do NOT need to submit items B - D referenced in these instructions.
 
SENIOR PSYCHOLOGIST: (Licensed for at least fifteen years)
Request the state(s) in which you are or were licensed in to send verification of your license and  copies of letters/forms from former supervisors documenting your pre and post doctoral supervised work experience directly to the Board's office.  Fill out the Application form through item #5.  You DO NOT need to submit items B - D referenced in these instructions.
 
IF YOU ARE CREDENTIALED BY THE NATIONAL REGISTER:
Request the National Register to send a copy of your entire file and a verification letter of your credential directly to the Board’s office.  You only need to fill out the Application form through item #5 if you have been licensed in another state for over 5 years.  If this applies, you do not need to submit items B - D referenced in these instructions.
