Overview: This Partnership's efforts to identify trends and reduce injuries and
illnesses in the state's logging industry include producing and distributing a logging
safety manual, conducting training programs, supporting logging safety programs, and
promoting increased employer action and responsibility in safety and health management. More
than 242 employers and 2,400 employees are affected by this Partnership.
Results: From 1998-2000, during a 16-month period prior to this
Partnership’s implementation, there were 8 logging fatalities in Louisiana. In 2003, there
were three fatalities, and in 2004, there was one. This rate is expected to remain low and
ultimately reach zero as a result of this Partnership and the increased awareness of safety
and health issues in this industry. |

Overview: There is a high incidence of employee injuries in nursing homes,
particularly musculoskeletal injuries/disorders related to patient handling, slips, trips,
and falls. The partners are working cooperatively to reduce these injuries and increase
awareness about muscular injuries, bloodborne pathogens, tuberculosis exposure, and
workplace violence.
Results: This Partnership has 210 participating employers and has
taken several positive steps towards reducing injuries and illness in the industry. The
Office of State Consultation visited each of the facilities to assist in hazard
identification and abatement, which should improve self-inspections at the facilities.
Additionally, OSHA conducted recordkeeping classes in three cities to improve the
facilities’ ability to track and trend injuries and illnesses. |

Overview: The Associated Builders and Contractors, Inc (ABC) and
its member companies; QUION, a chapter of Associated General
Contractors of America (AGC) and its member companies; and OSHA, Dallas
and Fort Worth Offices recognize the importance of ensuring a safe and
healthful work environment in the construction industry. Therefore,
they have partnered together to improve safety and health at their
construction sites. Goals include annual reduction of 3% in injuries,
illnesses and fatalities resulting from falls, struck by,
caught-in/between and electrocution, implementation of Safety and
Health Management System, decrease workers compensation cost, and
safety training.
Results: During FY07 four (4) members entered into
the partnership agreement, they are Charter Builders, LTD., Rogers
O’Brien Construction Company, Bob Moore Construction and TAS Concrete
Construction. Each new member developed and implemented a safety and
health management system which reflect the 1989 guideline. Three
10-hour Construction Safety and Health Courses were conducted in
English and two in Spanish. There were eight verification inspections
performed, in addition there were over 900 self inspections performed
with an average of 2.4 hazards per inspection, all were abated. |

Overview: This partnership agreement will be between the Associated
General Contractors of America, San Antonio Chapter and OSHA Austin Area Office. Implementation of
this partnership is expected to result in decreased serious injuries, illness and deaths in the
construction industry by improving safety and health management systems. This OSP will focus on
the four leading causes of death on construction sites (falls, electrical, struck-by, and caught
in/between).
Results: The partnership added two members during the evaluating period,
bringing the total number up to twelve. The aggregate Total Case Incidence Rate (TCIR) is 20%
below the BLS national average. Partners combined to perform 486 self-inspections that identified
and abated more than 2,600 hazards. OSHA assisted in conducting two OSHA 10-hour courses for
partnership supervisors, and AGC conducted eight topical training courses for employees, including
scaffolds, excavations, and first aid. |

Overview: The Partnership between OSHA ‘s Baton
Rouge Area Office and ABC’s Bayou Chapter targets a reduction in the DART rate
of construction sites within Louisiana. The partnership will concentrate on
reducing injuries, illnesses and fatalities in the four most common
construction hazards – falls, struck-by, electrical and caught-in- between.
This partnership also consists of a mentoring component in which companies
seeking to be mentored will be matched with another member company willing to
share knowledge and expertise regarding safety and health.
Results: The partnership now has five active
employers and 950 employees being impacted by the partnership. The DART Rate
is now 64 percent less than the previous year and 71 percent below the BLS
Rate. Before participation in the partnership is allowed, the member completes
an application which evaluates whether the applicant has a safety and health
program and rates the program based on standardized questions. The results
from this review by the National ABC provide them with a level of
participation ranging from Bronze, Silver, Gold and Platinum.
The Baton Rouge Area Office, CAS provided an OSHA update presentation covering
industry related topics of interest such as, upcoming training activities,
revised/proposed standards, hazard recognition and crane safety. Partners
utilized CAS availability to assist with the interpretation of standards,
recordkeeping requirements and to request OSHA publications for site
distribution. |

Overview: This construction partnership agreement is an effective tool
for ensuring safety at the Cowboy Stadium Construction Project. It will serve to establish a
cooperative effort in ensuring safety and maintaining an open line of communication between OSHA
and contractors on the worksite. The participants aims to create a "zero lost time accident" rate,
and a model safety and health management program that coordinates among the different contractors
and the use of safety and health professionals. This partnership agreement was developed jointly
by OSHA, Manhattan, Rayco Incorporated and 3i Construction LLP. Rayco and 3i Construction are
minority-owned, strategic alliance partners with Manhattan. The common objective and goal of the
agreement is to provide a safe and healthful work environment for employees involved in the
construction industry and to help prevent serious accidents and fatalities within the industry
through increased training, implementation of best work practices, enhanced safety and health
programs, and compliance with applicable OSHA standards and regulations.
Results: The Cowboys Stadium Partnership was signed in July of 2006.
Between July 2006 - July 2007, they have worked more than 1,486,520 hours with only five Days away
from work restricted and transferred activity cases. This is about 80 percent below the national
average for that SIC Code (1542) and NAICS Code (233320). The Cowboys Stadium project expects a
total of 5,000 employees will work on the project, with up to 1,500 employees onsite at one time
during peak activity. Eighty percent of the workers are non-English speaking Hispanics, so the
OSHA 10-hour course is provided in English and Spanish. Other courses offered on site by outside
sources included the following: Crosby 4-Hr. course (Rigging-Housing), CPR/First Aid
Certification, Aerial Work Platform Training Course and Fall Protection Training. All safety and
health training is being provided at no cost to the subcontractors. The partnership was signed on
July 17, 2006. |

LLC and participating contractors working on the Perkins Road Expansion
Project to enhance workplace safety and health through the strategy of
implementing fully written safety and health programs. The overall goal will
be to reduce serious injuries and illnesses by one percent per year and to
reduce the occurrence of the four major hazards in the categories of falls,
electrocutions, struck-by and caught-between by ten percent per year.
Employers who are willing to accept this program will receive OSHA assistance
and annual on-site enforcement inspections (in accordance with approved
protocols) in lieu of programmed inspections.
Results: This partnership has three active employers and 1600 active
employees. During the last year the companies had over 64 training sessions
for the employees and supervisors. The areas covered were hazard communication
standard and review of MSDS, and OSHA’s Big Four Focus Hazards in
construction, falls, struck by, electrocution and caught in between. Thirteen
self inspections were performed and all hazards identified were abated. The
injury and illness rate is 60 percent below the BLS Rate. |

Overview: Continuing efforts to enhance workplace safety and health
through a strategy of implementing written comprehensive safety and health systems, OSHA and the
Associated General Contractors of America (AGC)- San Antonio Chapter, renewed a safety and health
partnership on February 28, 2008. The overall goal of the partnership agreement is to reduce or
eliminate serious injuries and illnesses. The partnership will continue to focus in particular on
construction industry hazards such as: falls; electrocutions; struck-by and caught-between.
Measurements will be obtained from OSHA 300 logs and self inspections. Inspections and audits will
be conducted as frequently as deemed necessary by the company, but in no case less than once every
month. The partnership provides for: utilizing the services of the AGC Safety Representative or
their designee to perform these inspections and audits in addition to their own inspection and
audit; planning and preparing for emergencies; and conduct training and emergency drills, as
needed, to ensure that proper responses to emergencies will be "second nature" for all persons
involved. The employer shall instruct each employee in the recognition and avoidance of unsafe
conditions and the regulations applicable to his work environment to control or eliminate any
hazards or other exposure to illness or injury.
AGC is a national trade association for the construction industry with approximately 500 member
companies in San Antonio. The partnership, originally signed in 1999, represents a continuing

Overview: On April 17, 2008, OSHA, through its
Little Rock Area Office and CDI Contractors, LLC (CDI) signed a comprehensive
safety and health partnership agreement to provide safe and healthful working
environments for employees of CDI Contractors, LLC and the contractors they
may hire in the State of Arkansas.
Goals of this agreement include: implementing and maintaining effective Safety
and Health Management Systems (SHMS); reducing or eliminating injuries and
illnesses involving CDI employees, and others on CDI worksites; and
endeavoring to maintain a DART Rate equal to or less than the current Bureau
of Labor Statistics (BLS) rate for general contractors NAICS. The partnership
will focus in particular on construction industry hazards such as: falls;
electrocutions; caught in-between and struck-by.
The partnership is scheduled to terminate on or about April 17, 2011. |

Overview: On May 5, 2008, the Occupational Safety and Health
Administration (OSHA) and the Safety Program Specialist (SPS) Safety Association signed a
partnership agreement to reduce or eliminate fatalities and serious injuries in the construction
industry. The partnership will focus in particular on construction industry hazards such as falls;
electrocution; struck-by Trenching and excavation and caught in. These hazards increase in
complexity in typical multi-story worksites or large complex interior renovation type projects.
The SPS Safety Association supports the efforts of OSHA towards the reduction and/or elimination
of these hazards and recognizes the importance and value of leadership’s commitment in this
regard. To facilitate OSHA’s goal of reducing occupational illnesses and injuries, OSHA and SPS
Safety Association agree to the joint implementation of this partnership at all projects that also
include SPS sub contractors.
The goals of this partnership include: a Total Case Incident Rate (TCIR) and Days Away Restricted
and Transferred (DART) below the 2005 National BLS rate for NAICS construction code; develop a
program to assist Spanish speaking workers (at selected projects); and understand and follow
Safety and Health Management System (SHMS) requirements. |

Overview: On May 21, 2008, OSHA and the Audubon Bridge Constructors, a
joint venture of Flatiron Constructors Inc.; Granite Construction, Inc. and Parsons Transportation
Group, signed a safety and health partnership agreement to address construction and roadway
workzone industry hazards during the construction of the John James Audubon Bridge. Goals of this
partnership include: encouraging the Audubon Bridge Constructors and the subcontractors to improve
their safety and health performance and working toward reducing or eliminating safety and health
hazards through implementation of Safety and Health Management Systems. The partnership will focus
in particular on roadway workzone and construction industry hazards such as: falls; electrical;
caught in-between and struck-by.
The John James Audubon Bridge project is a new Mississippi River crossing between Pointe Coupee
Parish and West Feliciana Parish near Baton Rouge, LA. The project will include: a 2.44 mile
four-lane elevated cable-stayed bridge structure with two 11-foot travel lanes in each direction
with 8-foot outside shoulders and 2-foot inside shoulders; approximately 12 miles of two-lane
roadway; and four new intersections to existing roadway.
The project, when completed, is proposed to be the longest cable-stayed bridge in North America.
The project is expected to be completed during the summer in 2010. |
