It’s Time to Start Thinking About Back-to-School
The 7th Grade Yellow Immunization Card – A Must by the First Day of Class

HONOLULU – Young people need a shot in the arm to start 7th grade! By the FIRST DAY OF school, all 7th grade students must show proof of the following vaccinations:

  • 3 Hepatitis B
  • 2 MMR (Measles, Mumps, and Rubella)
  • 1 or 2 Varicella (Chickenpox)

If these immunizations are not completed by the first day of class, then the student must show documentation that an appointment is scheduled to complete the required immunizations. Without either of these documents, the student will not be permitted to attend school.

The completed 7th grade Immunization Record (yellow card) is required by State Law for entry into school. To avoid having students excluded from the first day of class, public health nurses, health aides, and registrars have notified families whose children are not up-to-date. Students are encouraged to turn in their yellow cards to their 7th grade school office over the summer.

Families who do not have a doctor and/or are uninsured and wish to locate an immunization clinic in their neighborhood, should call Aloha United Way by simply dialing 2-1-1, 24 hours a day, 7 days a week.
