Mucormycosis is comparatively rare in HIV as the predominant predisposing factor for this infection is neutrophil dysfunction. It usually requires total surgical excision for cure. We present a 37 year old previously asymptomatic man (CD4 = 10) who presented with an abrupt onset of unilateral sinusitis refractory to antibiotics which progressed to periorbital oedema. He was treated with intranasal antrostomy and ethmoidectomy. Apophysomyces elegans was isolated and he was treated over 2 months with various formulations of Amphotericin B. External ethmoidectomy with Caldwell-Luc procedure and a revision intranasal antrostomy were performed. His sinusitis continued to progress and he died 11 weeks later. This case highlights the aggressive course of invasive fungal infection in the immunocompromised host and the ineffectiveness of available antifungal agents. Even aggressive surgical debridement in this case was not successful.
