OBJECTIVES: To determine the HIV-1 prevalence and socio-demographic and behavioral risk factors for HIV-1 infection in female CSWs in N. Thailand frequented by members of a cohort of military conscripts. METHODS: Female CSWs (n = 306) working in brothels and other establishments in 6 districts in N. Thailand were studied by interview for HIV-1 risk factors. Sera were tested for HIV antibodies by ELISA with WB confirmation. Data were analyzed by chi-square and MH chi-square. RESULTS: HIV-1 prevalence was similar in younger CSWs, i.e., age 21 yrs. or less, (66.7%) and older CSWs (63.4%), but increased with longer duration of employment, 1-12 mos (47.1%), 13-24 mos (75.4%), 25-48 mos (80.7%), > or = 48 mos (60.8%) (MH X2 = 4.2, p = 0.04). Med. no. of customers/night was similar in HIV pos (5) and HIV neg (4). Hx of STD in past yr. was frequent and often associated with HIV-1 positivity, i.e., syphilis HIV+ = 19%, HIV- = 10%, p = .08; genital warts, HIV+ = 27%, HIV- = 9%, p = .001; genital ulcers, HIV+ = 29%, HIV- = 14%, p = .01; Condom use in the past month with > or = 80% of customers was reported by 85% of HIV neg and 71% HIV pos CSWs (X2 = 5.8, p = .02). CONCLUSIONS: Despite high rates of reported condom use with CSWs, HIV-1 and STDs are common because of high HIV-1 rates in brothel customers. More effective condom use and STD Rx are needed to control this epidemic.
