Risk factors for HIV seropositivity among IV drug abusers (IVDA) in France: evidence of a change in behaviour.

OBJECTIVE: To evaluate risk factors for HIV contamination in an homogeneous population of IVDA not recruited on the ground of health problems. METHODS: We have studied 152 consecutive IVDA entering LYON's prisons after november 1987 and 2 control groups of 100 subjects each (non IVDA and inmates denying drug abuse). RESULTS: The overall HIV seroprevalence among IVDA was 15% whereas none of the controls were positive. In a multivariate analysis the only variables related to HIV seropositivity were the form of needle sharing (NS), the location of drug addiction (Areas of low versus high prevalence of AIDS), and sexual promiscuity evaluated on syphilis seropositivity. We observed a significant decrease in NS between IVDA whose addiction had begun before and after 1986. That change was associated with a decrease of HBV (77 to 39%), HIV (18 to 8%) and syphilis (14 to 2%) seropositivity as well as the incidence of clinical hepatitis (44 to 19%). In a multivariate analysis a change in initial NS and rates of HBV and syphilis seropositivity significantly differentiated IVDA whose addiction had begun before and after 1986. Furthermore a reduction in NS was observed in 61% of IVDA of the first group. CONCLUSION: sexual promiscuity as well as NS may be responsible of the spread of HIV among IVDA. The change in sexual behaviour observed since 1986 was not an increase of the use of condoms but rather a reduction of the number of sexual partners.
