Risk factors interrelated between AIDS and STD among female prostitutes Mexico.

OBJECTIVES: Analysis of risk factors for HIV-1 infection and other sexually transmitted diseases (STD) and their interrelation among female prostitutes in Mexico. METHODS: Results from surveys on a group of 1,066 female prostitutes in 6 cities of central Mexico were analyzed. The studied variables were sociodemographic characteristics, sexual practices, prior sexually transmitted diseases (STD), use of condom, genital examination. Blood samples were taken to process anti-HIV-1 (ELISA and WESTERN BLOT), HBsAg (ELISA), anti-T, pallidum (VDRL and FT Abs), anti-Herpes IgM (ELISA), and cervical samples for processing N.gonorrhoeae Ag (ELISA) and C. trachomatis Ag (ELISA). RESULTS: Sociodemographic characteristics: Age (27 years), schooling (elementary level or less 78%), unmarried (51%). Sexual practices and prostitution: Average time for engaging in the practice of prostitution, 4.6 years; average number of sex partners per month, 45; use of some type of contraceptive, 83%; use of IV drugs, 0.5%; use of condoms during the last month, 75%. Antecedents of STD and examination; Have had some type of STD (50.3%); presence of vaginal secretion (38.6%); current genital ulcers (15.9%). Infection of HIV/STD; anti-T. pallidum (27.8%), C. trachomatis Ag (14.4%); N. gonorrhoeae Ag (12.7%); anti Herpes IgM (12.1%); HBsAg (5.7%); anti-HIV-1 (0.7%). Risk factors that were consistently observed were associated with infection of HIV-1 and other STD's. These factors were the following: less use of condoms, and more than 50 sexual partners per month. The odds ratio were between 2.5 and 4.2 with variable confidence intervals. DISCUSSION AND CONCLUSIONS: Female prostitution in Mexico is not associated with intravenous drug addiction; the prevalence of HIV-1 infection is low, the frequency of other STD's is high. Risk factors identified as: low rate of condom use; interrelation between HIV-1 and other STD's is seen with genital ulcers and C. trachomatis infection. Although the use of condoms by prostitutes is high, the transmission of HIV-1 can increase, and the frequency of STD's can serve as indicators for prediction as to what may occur in the next few years concerning HIV-1 infections.
