Persons infected with HIV who are moving into the symptomatic phase of their illness may present in a number a diverse ways. Early signs of immune deficiency include oral candidiasis and oral hairy leukoplakia. Pneumocystis carinii pneumonia may masquerade as hyperventilation/anxiety or may present in atypical form such as with pneumothorax or localised disease. Cytomegalovirus infection may produce protean manifestations including retinitis, fevers, or gastrointestinal ulceration. Kaposi's sarcoma continues to produce significant morbidity and mortality, and cerebral toxoplasmosis may present as headaches, siezures or personality change. Mycobacterium avium complex infection is being increasingly recognised and treatment and prevention strategies continue to evolve. A variety of other unusual presenting illnesses include bacilliary angiomatosis, HIV encephalopathy, cryptococcal infection, invasive aspergillosis as well as unusual manifestations of traditional sexually transmitted diseases.
