26 cases of acute retinitis necrosis (ARN) in 26 HIV infected patients.

AIM: ARN, described till 1985 in non immuno-suppressed patients, occurs frequently in HIV infected population. Our aim was to report 26 cases of ARN in 26 HIV infected patients and to compare these data to literature. METHODOLOGY: We have conducted a retrospective study from 5 ophtalmological departments (Paris, France). RESULTS: ARN was observed in patients with CD4 lymphocytes count under 100/mm3. Decreased vision was the first sign. Examination showed the characteristic triad: vitreous humor inflammation, arteritis and circumferantial afferent necrosis. In 60 to 90%, a viral vesicular eruption precedes of few days the ARN: in most cases VZV infection was concerned. Rarely, a neurological disease is associated. Bilateral (50%) blindness is the main complication (76 to 85%) due to detached retina. Our results suggest that VZV could be implicated in ARN. Diagnosis leads to an urgent anti-viral treatment. No drug is today efficient but foscarnet seems interesting, alone or in association (aciclovir, in situ ganciclovir).
