OBJECTIVE: The aims of our study was to describe the ultrastructural features of hairy leukoplakia (HL) and to confirm the presence of Epstein-barr virus (EBV) by immunohistochemistry and transmission electron microscopy (TEM). METHODS: Biopsy specimens of HL were taken from the lateral border of the tongue of 3 HIV-seropositive male patients belonging to stage IV C2 of the CDC classification and processed for routine histopathology, immonohistochemistry and TEM. RESULTS: The common histological features were a mild hyperkeratosis, parakeratosis and acanthosis of the epithelium, with characteristic koilocytotic prickle cells. In the upper epithelial layers the presence of EBV antigens was detected using an anti-EBV policlonal Ab in a three step immunofluorescence technique on frozen sections. At the ultrastructural level, ballooned keratinocytes were observed within the spinous layer. The infected cells were increased in size, with few cytoplasmic organelles and degenerated mitochondria; their pycnotic nuclei showed areas of condensed chromatin. The nuclei contained varying amounts of viral nucleocapsids. Some showed the central electrondense portion typical of herpes viruses. In our investigation the papilloma virus not detected at the EM level.
