Introduction -Computerised decision support is a potentially powerful way of implementing guidelines for chronic disease management. Current evidence from systematic reviews suggests that computerised decision support can have beneficial effects though there are few rigorous studies examining its impact on integrated computer systems and on chronic disease management.Methods - In a before and after cluster randomised controlled trial utilising a two by two incomplete block design, this study evaluates the impact of guidelines for the management of angina and asthma. General practices eligible to participate in the study were those with one of two specified computing systems with at least 50% of the doctors within them reporting using their computer system to review clinical data during consultations and for acute prescribing. The study is designed to have 80% power to detect a 10% difference in adherence to the guideline using a significance level of 5%. This assumes an intra-class correlation coefficient of the measures of process of 0.06. The study will have more power to detect changes in patient based outcomes. The data on the process of care will be collected by paper notes abstraction and by interrogation of computer systems, data on patient outcomes will be collected by a combination of generic and condition specific questionnaires delivered at three time points during the study. The study also contains an economic evaluation of an integrated case study. The results will be available for presentation at the conference.
