The quantitative relationships between antibiotic use in the hospital and emergence of resistance are unknown. This reflects the complexity of the relationship and the difficulty of obtaining reliable data regarding antibiotic use. MediMedia Information Technology (MMIT) obtains reliable drug use data at participating hospitals, then links drug use to DRG and ICD-9 information and provides summary reports to the hospital. SCOPE (Surveillance and Control of Pathogens of Epidemiologic Importance) is coordinated at the Medical College of Virginia Hospitals by Drs. Mike Edmond and Richard Wenzel. SCOPE analyzes isolates causing nosocomial bacteremias at approximately 40 participating hospitals. An alliance between MMIT and SCOPE has been formed to investigate the quantitative relationships between antibiotic use and resistance. At the ``hospital wide level, antibiotic use often does not have a significant relationship to the hospitals antibiogram, since a number of important variables are often not considered. These include infection control efforts, the lag time between use of an antibiotic and emergence of resistance, as well as a lag time between reductions in use and changes in hospital-wide susceptibility. A unique feature of the alliance is the ability to evaluate patient level data. Demographic data for each patient are known, as well as all antibiotics administered (from MMIT), both before and after a positive blood culture (from SCOPE). Infection control efforts can be included into these relationships and the emergence of resistance over time can be studied at a level not previously possible.
