Transmission of HIV infection has been documented in the health care setting. This roundtable will discuss real and perceived occupational risks in caring for HIV-infected patients, and the role of universal precautions in decreasing risk, current policies and practice surrounding the infected health care worker (HCW), and the risks of transmission to patients by the infected HCW. Data on the attitudes and practices of HCW's towards HIV infected-patients will be summarized. Discussion will also focus on differences in occupational risks between developed and developing countries and the feasibility of universal precautions in each setting. Emphasis will be placed on the costs of instituting universal precautions in comparison to the costs of selected or universal screening programs for hospitalized patients. Practical and ethical issues will be highlighted in the implementation of universal precautions or screening programs emphasizing the changing demographics of the HIV pandemic.
