I have consented to be interviewed for an oral history of the National Aeronautics and Space Administration (NASA).

I have no objection to the audio, photographic, or video recording of me, my voice, likeness, conversation and sounds for this project, nor to the use, exhibition, distribution and broadcast of my contribution to this project in any and all media throughout the world, nor to the use of my name, photograph, likeness, voice, or biographical material used in conjunction with this project, and for the incidental publicity or promotion thereof.

I understand that I will be provided an opportunity to edit the resulting transcript in order to clarify and expand my original thoughts, and that I will be provided a copy of the edited transcript for my own use.

I hereby transfer and donate to NASA any and all copyright and any and all other rights, title, and interest that might exist or that I might have in my contribution to this project. I also hereby transfer and donate any and all rights, title, and interest in and to any and all physical media in which such interview is fixed in a tangible form, including but not limited to paper transcripts, videotapes, audiotapes, and CD-ROMs.
