dealing with federal agencies. Please understand that while we can
assist you in bringing your case to the attention of the proper officials and
guide you through the process, we cannot direct an agency to decide a matter in
your favor.
In order for me to make inquires on your behalf, the Federal Privacy Act
requires your written authorization. To do this, please printout
the Privacy Release Form on this page, or contact my office at (602) 256-0551
to have one sent to you. Be sure to include your contact
information so that my staff is able to contact you promptly. Also,
please explain what type of assistance you are seeking from my office in the
space provided on the form and attach copies of any relevant documents or
correspondence.
Once you have completed
