A health plan or provider may disclose PHI to a health care provider for treatment, which includes ensuring continuity of care.

Example: A group home may disclose PHI to another health care facility to which the individuals will be evacuated in an emergency.

The minimum necessary standard does not apply.

NOTE: A covered health care provider may disclose to a third party that is not a health care provider for treatment purposes; minimum necessary does apply.
