Content CMS has available approximately thirteen million dollars in available funding to continue to support States' efforts to address complex issues in long-term care reform. CMS Releases the FY 2008 Real Choice Systems Change and Aging and Disability Resource Center/Area Agencies on Aging Grant Solicitation

 

CMS has available approximately $13 million in available funding to continue to support States' efforts to address complex issues in long-term care reform.  In addition to approximately $8 million in Real Choice Systems Change Grant funding, CMS was also awarded $5 million for Aging and Disability Resource Center/Area Agencies on Aging grants.

 

The FY 2008 Grant Category: Development and Implementation of a Person-centered Hospital Discharge Planning Model

 

The new grant solicitation will build upon the successes and direction of earlier Real Choice Systems Change and Aging and Disability Resource Centergrant opportunities by providing targeted assistance to States' in their efforts to improve hospital discharge planning through collaboration with, Aging and Disability Resource Centers(ADRCs), Area Agency on Agingand Centers for Independent Living.  Specifically, this grant opportunity for FY 2008 is designed to:

 

1.               Promote the development and implementation of enhanced hospital discharge planning models that meaningfully engage Medicaid-eligible individuals with disabilities (and their informal caregivers);

2.               Increase the capacity of existing , and develop new, single entry points (including ADRCs) to provide critical linkages to available long-term care services in the community and much needed supports for informal caregivers themselves;

3.               Inform CMS, other Federal agencies and Congress on national policy related to hospital discharge planning, person-centered planning, and caregiver assessment.

 

States, territories and the District of Columbia are eligible to apply for funding under this solicitation.  Applications are due no later than July 17, 2008 and must be submitted electronically on the grants website found below in the links outside of CMS.  The full solicitation can also be found in the links inside CMS below.
