Text File Instructions

The Office of Medical Assistance Programs fee schedule is named LVL3YYMM.
(YY =year, MM=month). The LVL3 file is updated quarterly. It is formatted as a fixed length text file (raw data) so that you can download it into a database program. This file is a self-extracting zip file. The input specifications for the LVL3YYMM file are:
