FSA/RMA Employees
  • AS-2005, Metropolitan Transit Promotion Program (MTPP) (Headquarters)
  • AS-2020, Implementing the Transit Subsidy Benefit Outside the National Capital Region (Field Offices)

 
FAS Headquarters Employees

 
  • AS-2008, Transit Promotion Program
  • FFAS-13, Metropolitan Transit Promotion Program Application.

 
Field Offices Cash Reimbursement

 
Eligible field office employees may complete AD-1170, to receive cash reimbursement when fare media is not readily available for a specific location.
Note: AD-1170 should be completed and forwarded to DOT through the Transit Coordinator at the end of each quarter and must be received at DOT by the 15th of the following month.

 
  • DOT Media Return Form (Transit Coordinators should use the DOT Media Return Form to return unused fare media to DOT.)

 

 
Pre-Tax Parking Benefits (Headquarters)

 
  • AD-1185, Pre-Tax Parking Application
  • Note: FAS employees use FFAS-13 in lieu AD-1185.
