This was the third annual gathering of organizations and individuals who are supporting those living with chronic conditions through Stanford’s Chronic Disease Self-Management Program (CDSMP), known in many areas of Oregon as"Living Well with Chronic Conditions."

Keynote Presentation: Self-Management and the Health Care System – Halsted Holman, MD (pdf)
The keynote speaker was Halstead Holman, MD, one of the original researchers and developers of the Chronic Disease Self-Management Program at Stanford University. Dr. Holman discussed ways to link clinical and community efforts to support self-management, sharing examples from around the country. Dr. Holman is a professor of medicine at Stanford University, a champion for patient-centered medicine and self-management.
