The American Association of Diabetes Educators (AADE) and
American Dietetic Association (ADA) have
developed a simple form for physicians to refer Medicare patients
with diabetes to a Medicare-certified diabetes educator for
diabetes self-management training (DSMT) and to a registered
dietitian for medical nutrition therapy (MNT).
