For Driver License, Commercial Driver License, Instruction Permit or ID Card
A name change can be part of an original, renewal or replacement transaction for a driver license, driver permit or identification card. You will need to visit a DMV Office in person, and will be asked to:
NOTE: Changing your name on your Oregon Driver License, Permit, or ID Card does not automatically change your name on your Oregon vehicle record, title or registration. You must apply for a name change for your Oregon title within 30 days of the name change. See section below for more information.
For Title & Registration
ORS 803.220 requires you to change the name on your title if your name changes by marriage or otherwise from that shown on the title. Changing your name on the title will also change your name on the registration.
To apply for a title in your new name you must:
- Submit the Oregon title;
- Submit a Request for Correction of Title Records;
- Make a statement on the form that you are the same person, such as "Jane Smith and Jane Doe are the same person due to... (marriage, divorce, court order, etc.)." It is also helpful to include your license, permit or ID card number and date of birth;
- Pay the title fee of $55; and
- Take the completed form with all of the above listed items to your local DMV office or mail it to:
