Your Date of Birth |
Your automatic Statement shows the date of birth you gave us when you first applied for a Social Security number or the last time you requested a replacement card. Your birth date is used to calculate both the number of credits you need to get Social Security benefits and the amount of those benefits, so it is important that your record has the correct date. If the date shown on your Statement is wrong, you must file a new application to correct your record. You can go to the application form SS-5 and instructions for completing and submitting it. If you requested your Statement, we show the date of birth you gave on your request form. |
