Here are some tips that will help you search for the information you need:

Step 1: You must first enter the ZIP code you wish to search in. When you have done this, click the “Submit” button.

Step 2: Select up to five categories by clicking on the associated checkbox. When you have selected your categories, click the “View Results” button.

Step 3: After clicking the “Vieiw Results” button in step 2, a "Search Results" page will be displayed. It will contain a list of suppliers, their phone numbers, and their address. For suppliers with more than one location, the number of locations will be listed in the address field. You can click this link to view a list of all the addresses.

In addition, the "Search Results" page will display a icon to the right of the names of suppliers who are Participating in Medicare. The physician names without a 'P' icon are Non-Participating Physicians. For more information regarding Medicare Participation, click here.

If you want a map of a specific location, or directions to that location, click on the 'View map and directions' link which is displayed beneath each location's address.

If you want to print the results of your search, click on the "Print This Page" icon. This will display a "printer friendly" version of the page. To return to the original page, click on the "Return to previous page" link at the top of the page.

If you want to view the Supplier Directory in a larger font, click on the "Use Larger Font" link located in the upper right corner of your screen. To return to the normal font size, click on the "Use Smaller Font" link.
