Prevalence of Efflux Pumps among Clinical Isolates of Multiple Antibiotic Resistant Pseudomonas aeruginosa.

BACKGROUND: The presence of multidrug efflux-pump systems in aeruginosa showing resistance to various antimicrobials is increasingly reported. We determined the frequency of efflux-mediated resistance in 1,014 recent clinical isolates of aeruginosa collected in a week in 136 hospitals in Spain.METHODS: Efflux pump profiles were based upon susceptibility to 12 antimicrobials (piperacillin, ceftazidime, cefpirome, cefepime, aztreonam, imipenem, meropenem, chloramphenicol, erythromycin, norfloxacin, ciprofloxacin and levofloxacin) and response to the broad-spectrum efflux pump inhibitor CCCP. Criteria for each pump profile was established in comparison with laboratory mutants with known genotypes and varied pump expression.RESULTS: Among the 1,014 isolates, 165 (16%) showed to be multiresistant (carbapenems and/or cephalosporins and/or quinolones and/or penicillins). The MexAB profile was the most prevalent among these isolates (77 or 46%), and the MexAB/EF was observed in 10.3% of isolates. Other phenotypes were: AB/ CD: 6%; EF: 4.2%; CD: 1.2%; AB/CD/ EF: 1.6%. The addition of 0.5 microg/ml of CCCP decreased from two- to 16-fold the MICs of imipenem, meropenem, ciprofloxacin, levofloxacin and cefpirome.CONCLUSIONS: These results show that Mex-type efflux pumps are highly prevalent among multiresistant aeruginosa isolates and that the activity of different antimicrobials can be potentiated by inhibition of efflux pumps.KEYWORDS: Efflux pumps; Multidrug resistance; Pseudomonas aeruginosa
