Quantitation of HIV-1 in genital tract secretions.

Quantitative PCR and culture methods have been optimized for assessment of HIV-1 titers in semen and cervicovaginal specimens. Proviral HIV-1 DNA is detectable in approximately 80% of semen samples, with copy numbers ranging from 20 to 10,000/ejaculate. Cell-free HIV-1 RNA is detected less frequently (40% of cases), with copy numbers ranging from 10 to 500. Likewise, the prevalence of HIV-1 culture from semen cells is higher (25%) than that of cell-free seminal plasma (4%). Similar HIV-1 titers are detected in cervicovaginal lavage specimens. The detection rate of proviral DNA by quantitative PCR is 66%, with copy numbers ranging from less than 10 to greater than 50,000 per sample. Cell-free HIV RNA is detected less frequently (20% of samples), and in lower copy numbers (up to 3,000/sample). The HIV-1 culture rate from cervicovaginal lavages is 33% from the cellular fraction and 3% from the cell-free fraction. HIV-1 titers in genital tract secretions vary considerably between individuals and within individuals over time. Factors associated with HIV-1 shedding into genital tract secretions include genital tract inflammation and advanced disease stage.
