Quantification of Human Herpesvirus-8 by Real-Time PCR in Leukocytes of AIDS Patients with Kaposis Sarcoma and Multicentric Castlemans Disease.

BACKGROUND: We performed a serial evaluation of the human herpesvirus-8 (HHV8) DNA load in leukocytes of AIDS patients on highly active antiretroviral therapy (HAART) with Kaposis sarcoma (KS) and those with both KS and multicentric Castlemans disease (MCD). METHODS: The number of HHV8 copies per 10[5] leukocytes was determined by quantitative-competitive real-time PCR (ORF26) using specific fluorescent probes in a LightCycler (Roche Molecular Systems). HHV8 serology was determined by two ELISA tests using lytic antigens (ORF65 and K8.1). RESULTS: Four blood samples were obtained at a 3-month interval for 6 patients with KS only and for 2 subjects with both KS and MCD. All subjects were seropositive for at least one of HHV8 lytic antigens. Among patients with KS only, the viral load was negative (< 50 copies) in all samples from 4 patients whereas it was weakly positive (maximum < 150 copies) in samples from 2 patients including one with progressive pulmonary KS despite HAART. In contrast, the viral load was > 1000 copies in at least one blood sample for the 2 patients with both KS and MCD. In one subject with MCD, the viral load became rapidly negative (47 210 to < 50 copies) within 10 days following aggressive chemotherapy with CHOP whereas the decrease was more progressive over 3-6 months (1080 to < 50 copies) following start of chemotherapy with bleomycine and vincristine in the other patient with MCD. No changes in HAART regimen or CD4 counts could explain the HHV8 load decrease in the latter 2 patients. CONCLUSIONS: 1-The pathogenesis of the two HHV8-associated diseases (KS and MCD) seems different since only MCD was associated with important viremia in our study. 2- A course of chemotherapy can decrease the HHV8 DNA load in the blood of AIDS patients with MCD.
