Customize this letter template to request that a local or state official issue a proclamation to declare the third week in September as Stay in Circulation Week in your community.  Consider inserting local statistics and facts to make your proclamation relevant to your community.


Dear [insert local official such as Governor, Mayor, etc.],

In the United States, more than 8 million Americans suffer from peripheral arterial disease (P.A.D.)—hardening of the arteries (also known as “atherosclerosis”) in the limbs, often the legs.  One in 20 Americans over the age of 50 has P.A.D., and many people with P.A.D. do not experience any symptoms.  P.A.D. can reduce mobility and increase the risk of heart attack and stroke.  This is an important public health issue for our community—adults over age 50 are especially at risk for P.A.D. and they may not know about it.  In our area, XX percent [If possible insert statistic about the over age 50 population in your area.] of adults are over the age of 50.

To bring more attention to P.A.D., the National Heart, Lung, and Blood Institute, in cooperation with the P.A.D. Coalition, created Stay in Circulation:  Take Steps to Learn About P.A.D., a national awareness campaign aimed at helping adults over the age of 50 learn about P.A.D.  [Insert Organization name] is supporting the initiative by hosting a series of local P.A.D. awareness-related events throughout the week.  This occasion serves as an ideal opportunity for [insert city/state] to join this important national public awareness campaign by supporting Stay in Circulation and helping to raise awareness in our [insert city/state] about this disease.

I am writing to encourage you to issue a Proclamation declaring the week of September [insert dates, year], as Stay in Circulation Week in [insert city/state].  P.A.D. is a serious disease that is on the rise among midlife and older Americans and stepping up awareness of P.A.D. will help adults in our community stay in circulation and remain healthy and active 

P.A.D. is caused by the same risk factors that lead to heart disease.  People who are at risk for P.A.D. include anyone over the age of 50, especially African Americans; those who smoke or who have smoked; and those who have diabetes, high blood pressure, high blood cholesterol, or a personal or family history of vascular disease, heart attack, or stroke.  P.A.D. is a common and treatable disease, but it is still largely unknown, often unrecognized, and regarded by many as an inevitable consequence of aging.  With your support, we can change that.

Whereas, [insert Organization] recognizes that more needs to be done in [insert city/state] to raise awareness about peripheral arterial disease (P.A.D.)—a disease that affects 8 to 12 million Americans, including many adults here in [insert city/state].

Whereas, one in 20 Americans over the age of 50 has P.A.D.

Whereas, P.A.D. is still largely unknown, often unrecognized, and regarded by many as an inevitable consequence of aging. 

Whereas, P.A.D. shares many of the same risk factors as heart disease.  People who are at risk for P.A.D. include anyone over the age of 50, especially African Americans; those who smoke or who have smoked; and those who have diabetes, high blood pressure, high blood cholesterol, or a personal or family history of vascular disease, heart attack, or stroke.

Whereas, the Stay in Circulation:  Take Steps to Learn About P.A.D. campaign seeks to increase public and health care provider awareness about P.A.D. and its association with other cardiovascular diseases—with the goal of ultimately increasing prevention measures and early detection among all Americans over the age of 50.

Whereas, early detection and treatment of P.A.D. can keep people active and healthier longer.  Leg health is critical to maintaining active and independent lifestyles.

Therefore, citizens in [insert city/state] need to take action to make leg health a priority for themselves and their families, become aware of their risk for P.A.D., and take action to control their risk.
