PHS-50 (PDF). Complete and submit
readable copy (photocopy acceptable) with original signature.

___2. | Download and have FOUR (4) Reference forms (PDF) completed for the references listed in section 21 of the PHS-50. The reference may mail to the address above or can provide it to the applicant in a sealed envelope to be mailed in with the application. |

___3. | Curriculum Vitae or Resume, including clinical rotations completed and planned. |

|
___4. | Download, complete and return Addendum (WORD) to Declaration for Federal Employment. The Job Title on the Announcement is “IHS Pharmacy Practice Residency.” The Announcement number will be added in by the Phoenix OHR office. |

___5. | Pharmacy school transcripts – original. |

___6. | Select IHS Application

If selected for a residency, an applicant is still required to meet the standards of either the civil servant or Commissioned Corps personnel system. Most applicants are interested in applying to the Commissioned Corps, a lengthy application process. If you are interested in becoming an officer in the USPHS regardless of whether you’re selected as a resident or would like to start the application process prior to the residency selection, please check the YES box on the Application Sites Form. Pertinent information will then be forwarded to the Office of Commissioned Corps Operations (OCCO). For additional questions on the different personnel systems, refer to the FAQ page, or discuss with one of the IHS residency program directors. |
