The Division certifies Outpatient Physical Therapy service providers. These are physical therapy services, including speech pathology services, furnished by a provider of services, a clinic, rehabilitation agency or public health agency to an individual on an outpatient basis. Businesses must apply for facility license BEFORE accepting patients License: $30 yearly.

The Division, under the direction of the Health Care Financing Administration (HCFA), also determines eligibility of the facility for Medicare and Medicaid reimbursement certification. Contact HFD for further information is you wish your facility to be eligible.
NOTE:  There is no special license for OCCUPATIONAL THERAPISTS. However, many of the services and practices conducted by occupational therapists are very similar to physical therapists. Occupational therapists that wish to ensure that they are exempt from licensure under the physical therapist statutes should write to the Physical Therapist Registration Advisory Committee for clarification. The letter should contain a detailed description of the services provided.
