Q: Where do I get an Oregon withholding allowance certification?
 
A: Oregon uses the federal W-4 form. Whatever an employee claims for federal purposes should also be used for Oregon purposes.
 
Q: Where do I get a W-2 and W-4 forms?
 
A: These are federal forms and should be requested from the IRS. Phone number 1-800-829-3676.
 
Q: I have an employee who claimed exempt on his W-4 form. What do I do?
 
A: Please send a copy of the W-4 to the Department of Revenue if:
  • an employee claims exempt on the W-4 form for state withholding purposes only, OR
  • the employee claims exempt on the W-4 form for both state and federal pruposes, AND the employee's earnings exceed $200 a week.
