How to request public records from the Oregon Department of Revenue
If you need a copy of your income tax return, do not use this public records request form; follow these instructions instead. 
 
We do not have copies of property tax statements; you must get those from the county in which your property is located.
 
To request public records from the Oregon Department of Revenue, you must:
  1. Complete the Public Records Request form.
  2. Sign and date the form.
  3. Submit to the address or fax number on the form.
We regret that we can't accept this form via e-mail or web-based submission.
 
Section A — Requester information
  • Asterisk fields are required.
  • Provide as much information as possible so that we may contact you if we have questions about your request.
Section B — Records requested
  • Asterisk fields are required.
  • In the "Description of records requested," be as specific as possible.

    Example: You are compiling information about the assessment and taxation programs of Oregon's smaller counties. You request "all county assessment and taxation program reviews and related documents."

    This could include preliminary drafts from every year that reviews have been completed, all related e-mails and memos, and other indirectly related documents you may not need.

    This broad of a request could be very expensive for you and difficult for the department to fulfill.

    Instead, consider narrowing your request to "final assessment and taxation program reviews, if any, on Oregon's four smallest counties between January 1, 2006 and January 1, 2007." It may save you time and money, and you may get more of what you need.
  • Tell us if you prefer to inspect the documents in person or receive copies, and how you'd like to receive them: e-mail, fax, or mail. This will depend upon the size of files and/or the number of documents in your request, and the types of documents. For example, some files are too large to e-mail or may not be suitable for faxing.
  • There may be a fee to complete your request.
Sign and date the form. We must have your signature to fulfill your request.

Many of the records we maintain are confidential taxpayer information and may be exempt from disclosure under Oregon's public records laws.
