Criminal Fines and Assessment Account Coupon for Municipal and Justice Courts
 
Replacement Coupons
 
Municipal and Justice Courts use payment coupons to report to the Oregon Department of Revenue the amount of funds collected for the Criminal Fine and Assessment Account (CFAA), Law Enforcement Medical Liability Account (LEMLA), and Intoxicated Driver´s Program Fund (IDPF).
 
We send each court coupons preprinted with the court´s name, address, and account number. If the preprinted coupons are misplaced or destroyed, replacement coupons may be printed from this website and used to accompany the court’s payment.
 
Procedure
 
There are four categories of funds reported on this coupon.
 
Unitary Assessment -
Moneys designated as Unitary Assessment as described in ORS 137.290.
Other State Fee -
Non-Unitary Assessment moneys outlined in ORS 137.295 (2)(c), applicable state fines for traffic offenses described in ORS 153.630, and other miscellaneous fines designated for deposit to the Criminal Fine and Assessment Account.
IDPF -
Moneys designated for the Intoxicated Driver´s Program Fund as described in ORS 813.030 and 813.240.
LEMLA -
Moneys designated for the Law Enforcement Medical Liability Account as described in ORS 137.309 (7).
If no funds are collected for one or more categories
 
Submit a coupon for the month even if no funds were collected. If there were no collections for a particular program, enter -0- in the box for each program. If there were no collections for any program, enter -0- in all the boxes including the Total box, and mail the coupon without a payment. Or send an e-mail to Criminal.Fine-Assessment@state.or.us and enter "-0- coupon" in the subject line. The e-mail text should include the court name and the month of payment was not collected.
 
Submitting -0- amount coupons in a timely manner helps avoid a late payment notice. It helps keep your account accurate and allows us to record future payments into the proper time period.
 
Payment due date
 
Send payment and coupon to the Oregon Department of Revenue by the last day of the following month the funds are collected (ie. funds collected in January are due by February 28.)
 
Delinquent dates
 
Payments become delinquent on the 10th of the month following the due date. If you send payments after this date, you may be charged interest.
